Student Applicants,

You may view the online components at any time without having to complete the form.  Click the backwards arrow on your browser or the application link to return to this application page.  For the online portion, we do recommend that you have all the information gathered before you start, so you can finish it all together in one sitting.

 

You also need to identify the teacher who will write you a recommendation.  We prefer that this be your chemistry teacher first or one of your other science teachers.  Additional recommendations are welcomed but not necessary.

 

You will need to have a valid email address to complete this application.  If you do not have one at home, consider asking your recommending teacher or guidance counselor at your school.  You may also go to any public library to use the internet and open a free gmail or yahoo account.

 

After you submit the online application, within 3 days you will receive an email confirming the information.  This email will be sent to the email you provided on the application.

 

The application consists of 5 parts,  the first 3 are online, and parts  4 and 5 need to be mailed by April 1 or brought to the required parent meeting on March 16 at IUPUI at 6:30 pmClick here for exact location, driving and parking directions.

 

Online

1.  General Information - online, click here.

2.  Academic Achievement  - you may need to look at your transcript for this, click here

3.  Teacher Recommendation -  to be completed by teacher, click here.

 

Paper Copies:  Mail to them at address  provided at bottom of form # 5.

 

4. Official School Transcript  -  schools make their transcripts 'official' in a variety of ways.  Some have a raised embossed seal, others simply make it official by sealing the envelope and placing a stamp across it.  Ask your guidance department to be sure!

 

5.  Financial Statement  - this information is required by our national sponsors and determines financial eligibility of students receiving a stipend.  Please complete and sign it even if you are NOT applying for stipend eligibility.  For more info, click here and read  Family Income Requirements  & Exceptions to Income Eligibility.

 

As noted, mail the paper copies to the address provided on form #5.  Applications  MUST be mailed & postmarked by April 1.
